Your family and friends might follow you on twitter, and sure, your author friends, too. But what about readers, are you getting any of those? If you're an unknown author, it's highly unlikely that you've attracted many fans. So how do you bring them in?
First. Set up your twitter page so that when potential fans come to visit, they can find out about your book. A link to your website, Facebook Fan Page, or your Amazon link would work. Your profile pick or cover art could also be the cover of your book.
Next. Figure out who writes in your genre that would have an established fan base similar to your ideal reader. If you write YA it might be Amanda Hocking, or if you write Sci-Fi, it might be Orson Scott Card. Then check to see if they have a twitter account. Take a peek at their followers.
Finally. Scan their followers for readers and reviewers and then follow ten or so of them. (Twitter doesn't like it when you follow too many at once.) When they get your follow request, chances are they'll peek at your profile. If your book appeals to them, they might make a purchase or at least follow you back.
